A case conference is a short, secure video meeting where their GP and other health professionals — specialists, allied health, mental health — come together to review their care and plan the best way forward.
The goal is to make sure every part of their health is looked after in a coordinated way — fewer mixed messages, fewer repeated tests, and a team that's genuinely on the same page.
Their GP coordinates the conference and decides which clinicians take part. Conference.care simply provides the secure platform that makes it easy to bring the team together.
